NEW STUDENT INFORMATION / INFORMACION DEL NUEVO ESTUDIANTE
Once the form above has been completed, the following documentation is required to complete your child's registration. Registration will need to be completed at your child's campus. Your child's campus will contact you to set up an appointment to complete registration.
Required Documentation:
Student's birth certificate.
Immunization Records
Proof of residence (current utility bill, lease agreement, or rental receipt)
Eligibility of lunch status (proof of income and food stamp letter if applicable)
Parent/Guardian photo identification.
Additional Information:
Social Security numbers are NOT required. However, if available, Social Security Numbers are requested to be used for identification purposes.
All eligible students must report to their zoned schools to register.
